The form is used by a member to identify the type of assistance needed, as follows:

nFirst, the upper part of the form is filled in and signed by the member, stating the assistance requested.

nSecond, the form is presented to the Relief Committee (or other appropriate committee of the lodge) for an investigation.  The committee needs to establish that the member is in good standing, that the request reflects a legitimate need, and that a real financial hardship exists.

nThird, the committee’s conclusion is presented to the lodge for a vote, the results of which must be attested by the presiding officer and by the secretary, with the seal of the lodge.

nFourth, the form is sent to the California Visual Research Foundation.  It can be mailed in care of the office of the Grand Lodge of California in Saratoga or given to any Board member.

nFinally, the Board of Directors reviews each request, does any additional investigation that is needed, and arranges for the assistance if it is determined to be appropriate.
